About the event

Is your child nearing the end of paediatric care? Join us to find out more about what the options are once they turn 18 and move into the adult healthcare system.

This information session is for parents, carers and people supporting teenagers with Down syndrome.

Participants will gain knowledge about:

  • navigating the transition from paediatric care to adult healthcare
  • what to consider and how to prepare
  • the services available
  • how the Transition Support Service at Royal Children’s Hospital can assist with the transition.

Evelyn Culnane, Royal Children’s Hospital

Evelyn Culnane leads the Transition Support Service at the Royal Children’s Hospital in Melbourne. She and her team care for young people with complex medical conditions and disabilities, transitioning from paediatric to adult health services across all medical and surgical units. For over 15 years, Evelyn and her team have helped prepare patients and their families for this change and championed new and innovative pathways to adult services.

Evelyn is passionate about ensuring a strong consumer voice, has authored and co-authored several research publications and provides consultation to other health services nationally and internationally.
